Fertility Treatment in Japan

Japan performs more ART (Assisted Reproductive Technology) cycles annually than any other country, with over 498,000 cycles per year. In a landmark policy shift in April 2022, the government brought IVF, ICSI, artificial insemination, and embryo cryopreservation under national health insurance coverage for women under 43, covering 70% of treatment costs. Patients now pay roughly $1,500–$3,000 USD out of pocket per insured IVF cycle, compared to $5,000–$10,000 before the reform. Women under 40 can receive coverage for up to 6 cycles, and those aged 40–42 for up to 3 cycles.

Third-party reproduction is heavily restricted in Japan. There is no specific law governing egg or sperm donation, but JSOG (Japan Society of Obstetrics and Gynecology) guidelines effectively limit donor-assisted reproduction to legally married heterosexual couples. Egg donation is discouraged by JSOG and virtually unavailable domestically — most patients travel abroad. Sperm donation exists but has sharply declined after a 2020 civil law allowing donor-conceived children to learn donor identity at age 18. Only a handful of clinics maintain active sperm banks. Commercial gamete donation and surrogacy are prohibited.

Despite these regulatory limitations, Japan is renowned for its world-class IVF technology — particularly natural-cycle and minimal-stimulation protocols pioneered by clinics like Kato Ladies Clinic. The country offers four main fertility hubs: Tokyo (largest concentration), Osaka (western Japan), Nagoya (central), and Fukuoka (Kyushu region). English-speaking support is increasingly available at major clinics, though some charge additional fees for language assistance.
