Austria underwent a landmark shift in reproductive medicine in 2015 when the Fortpflanzungsmedizinrechts-Änderungsgesetz (FMedRÄG) came into force, legalising egg donation, sperm donation for unmarried couples, and same-sex female couples' access to assisted reproduction for the first time. Previously one of the most restrictive countries in Europe on these matters, Austria now operates a regulated, identity-release-only donor system: all donors — both egg and sperm — must agree to have their identity disclosed to any donor-conceived child upon reaching adulthood. This framework prioritises transparency and the rights of donor-conceived people, and has been praised by patient advocates across Europe.
Austria's IVF Fund (IVF-Fonds) is one of the most generous state subsidy programmes in the EU. Qualifying patients receive 70% of treatment costs reimbursed by the government for up to four stimulated cycles. Eligibility requires that both partners be Austrian health-insurance members, the woman be under 40 (for own eggs) or under 45 (for donor eggs in some cases), and the treatment be performed at a licensed IVF-Fonds centre. After the Fund subsidy, patients typically pay EUR 3,000–6,000 per cycle out of pocket — far below the unsubsidised European average of EUR 8,000–12,000. Austria's three primary fertility hubs are Vienna (home to the largest concentration of clinics), Salzburg (a gateway for German cross-border patients), and Innsbruck (serving the Tyrol region and northern Italy).
Egg donors must be aged 18–30 and sperm donors 18–40. Anonymous donation is not permitted; all donations are altruistic and identity-release. Surrogacy — gestational or traditional — remains prohibited under Austrian law. LGBTQ+ access has expanded since 2015: lesbian couples and single women can access IVF with donor sperm, and the law was further updated to extend these rights without a medical infertility diagnosis requirement. Austria's clinics are licensed and inspected by the Bundesministerium für Soziales, Gesundheit, Pflege und Konsumentenschutz and must meet strict laboratory and clinical standards.
